I/we understand that the Blazing 7 on 7 and Aideum Event Marketing (defined below) assume no responsibility for accidents, injuries, illnesses, death, loss or damage, which my child/children may sustain as a result of his/her participation in any sports or athletic activities, the use of any equipment, or any other activity which will be conducted during the Blazing 7 on 7 “Programs”. I expressly acknowledge and agree that I/we assume the risk for any and all accidents, injuries, illnesses, death, loss or damage, which may result from my child’s/children’s participation in the “Program.”
In consideration of the privilege of participating in this program, I/we hereby voluntarily release, discharge and hold harmless the “Program”, Blazing 7 on 7, and their employees or agents and any other organization which contributes to the operation of this program, from any and all claims for accident, injury, illness, death, loss or damage, which my child/children may suffer as a result of his/her participation in the “program”.
The “Program” will make every attempt to provide positive and reasonable accommodations for its participants; however, any children who are: (1) a danger to themselves; (2) a danger to others; or (3) a disruption to the normal activities making it difficult for other children to enjoy the “program”, will be considered for dismissal from the “program”. Any participant who is dismissed from the “program” will not be refunded any fees paid.
I understand that no accident or medical insurance is provided with this program. I hereby give permission to the staff to provide minor medical assistance to my child, such as attending to cuts and scrapes. In the event of an emergency medical condition, the program staff will notify me as soon as possible. If for some reason, I/we cannot be reached, it is understood that the staff will immediately contact 911 for professional assistance. Any expense incurred with the dispatch of emergency personnel will be my responsibility and I/we will hold the program harmless from any expense incurred. I further hold the program harmless from any services provided by the emergency personnel.
I give my permission for my child/children to participate in all program activities, participate in field trips and to ride in authorized vehicles for the purpose of transportation in connection with Program activities.
